The questionnaires on this site are used in healthcare to assess symptoms. Here you can fill them in and see how the total score is calculated. The total score is not a diagnosis and does not tell you whether you have an illness. We only show the total score and the questionnaire’s reference table, and the advice about support is the same whatever your total score.

The site does not replace healthcare or contact with healthcare professionals. Contact your health centre or call 1177 if you need advice. If there is immediate danger, call 112.

The questionnaires are reproduced with source references. You can find the sources and the questionnaire’s reference table alongside the total score.

No guarantee

We try to keep the content accurate and the site available, but we cannot guarantee that everything is always free of errors or that the site always works. A self-assessment cannot determine what help you need.
