What is the PHQ-9 questionnaire?

PHQ-9 is a nine-question measure of depressive symptom severity. The questions cover the past two weeks. The PHQ measures were developed by Robert L. Spitzer, Janet B.W. Williams, Kurt Kroenke and colleagues.

How the questionnaire is scored

Each answer is scored from 0 to 3. The nine scores are added to give a total score from 0 to 27. The additional question about difficulties in daily life is not included in the total score.

How it is used

The instruction manual describes PHQ-9 as a measure of symptom severity and change over time. A clinical assessment is needed to determine whether a disorder is present. This site only shows the total score and the published reference levels.
